Pass/Fail application
This form is used to request that the grading basis of a course be changed from regular (A-F) grading to Pass/Fail grading. Only students pursuing an undergraduate degree in the College of Arts and Sciences—which includes the Media School, Hamilton Lugar School of Global and International Studies, and the Eskenazi School of Art, Architecture, and Design—should use this form.
Students should review the College's Pass/Fail policy and consult with their College of Arts and Sciences academic advisor before completing this form.
Forms must be submitted to the College by 11:59 p.m. Eastern Time of the official Pass/Fail deadline for the semester and session.
